ADMISSIONS

PLEASE SEE BELOW FOR ITEMS AND DOCUMENTS REQUIRED PRIOR TO ADMISSION

INPATIENT TEAM

  •  Please provide a list of current medications with the doctor’s signature or electronic signature (E-sign).
  •  Discuss medication options with the attending physician or healthcare provider.
  •  Ensure that prescriptions are sent to our pharmacy at least 24 hours before intake. Medications must be delivered prior to admission.
  • Members should arrive with a minimum of a 14-day supply of medical or mental health medication.
  • A recent TB test result is required before admission.
  • Please provide a facesheet containing essential demographic and insurance information.
  • Submit a comprehensive history and physical (H&P) report completed by a healthcare professional.

PROVIDER

  • The admission orders necessitate the provider’s signature.

CLINICAL TEAM

  • Provide the name of the psychiatric provider and schedule the 7-day Behavioral Health Management Plan (BHMP) appointment.
  • Ensure that a recent TB test result is submitted before admission.
  • Please provide a copy of the insurance card for verification purposes.
  • Submit documentation containing the diagnosis and relevant medical codes.
  • If applicable, provide contact information for the guardianship point of contact.
  • If applicable, provide documentation related to court-ordered treatment.
  • If the member has a payee, please provide contact information.
  • Ensure that the member arrives with at least 7 days’ worth of clothing and personal care items.

The items listed are required for admission. Request the outpatient case manager to provide the current treatment plan annual assessment.

CLINICAL TEAM

  • All admissions documents require signatures.
  • Identification is necessary if available.
